Scheduling a New Appointment
Appointments can be scheduled by calling our office. All appointments are scheduled in advance including chemotherapy, injections, lab visits and doctor visits.
If you have an urgent medical problem, same day appointments are available by contacting the office.
Preparing For Your First Appointment
When you call to schedule your first visit, please be prepared to provide information regarding your diagnosis, referring physician and insurance. The forms that need to be filled out are available online in the Forms section to the left. We request that all medical records pertaining to your diagnosis be forwarded to our office at least 48 hours in advance of your appointment. Your doctor will review these documents as he prepares for your visit.
In an effort to allay as many worries as we can about your treatment, our financial counselors will meet with you to discuss the cost of your treatment, what your insurance company will and pay and what, if any, financial obligation you may incur. Payment options and patient assistance programs may also be discussed. If your insurance plan requires that you make a co-payment, that payment is due at the time of service. We accept cash, checks and credit cards.
